• Injury Information: we must collect information about the injury for which we are providing you with treatment for, this information is collected from you, other professionals involved in your treatment, and from our own observations during the course of your treatment.
• Medical History: we often must collect information about any previous injuries and medical conditions which you have suffered from.
• Health Questionnaire / PARQ Information: when we are undertaking an assessment of your health background and lifestyle, we may collect information on your: medical history, activity levels, medication history, occupational history, sport and exercise history, smoker status, alcohol consumption, family health history, vaccination history, previous general health history, and general health concerns. This information is necessary to allow us to properly treat any injury which you have, and to develop a suitable health and fitness programme.
• Medical Professionals: we may collect details of any other medical professional who is, or has previously been, involved in your care (e.g. your GP).

When we are processing your Special Category personal data, we are relying on the lawful basis that such processing is necessary for the purposes of medical diagnosis and the provision of healthcare. 

How we collect your personal information

We use different methods to collect data from and about you including through:

• Direct interactions: We may receive personal data directly when you:
  • meet us in person and fill in any forms and/or questionnaires which we provide to you
  • fill in any forms on our website or via email
  • book an appointment on our website
  • purchase items from our online store on our website

  • give us some feedback.
• Third party medical professionals: We may receive information about your injury and/or your medical history from other medical professionals (e.g. your GP) where you have given us or them your permission to share your records for the purposes of treating your injury.

If you fail to provide us with your personal data, we may be unable to properly treat your injury, or provide you with any follow up information which you have requested.

How we use your information

We use the information we collect in various ways, including:

• to develop and provide healthcare rehabilitation and treatment services to you (e.g. diagnosing; treating; and rehabilitating your injury, providing you with advice, developing an appropriate treatment plan for your specific injury and circumstances
• to develop health and fitness exercise programmes and advice to you, fit to your background, and effective for your ability and goals
• if ever necessary, to advise your GP of the details of your treatment or obtain from your GP, details relating to your physical or mental health or condition, either before the commencement of your treatment, at any time during the course of your treatment or after the end of your treatment, subject to you providing us with your consent to do so
• to connect with you as a patient and build ongoing relationship with you as necessary
• Health Questionnaire / PARQ Information is used as necessary to assess your health status, and to develop a tailored health and fitness programme in connection with any occupational health programme
• Provide, operate, and maintain our website
• Improve, personalize, and expand our website
• Understand and analyse how you use our website
• Develop new products, services, features, and functionality
• Communicate with you, either directly or through one of our partners, including for customer service, to provide you with updates and other information relating to the website, and for marketing and promotional purposes

Log Files

SportSciMed follows a standard procedure of using log files. These files log visitors when they visit websites. All hosting companies (in our case GoDaddy) do this and a part of hosting services' analytics. The information collected by log files include internet protocol (IP) addresses, browser type, Internet Service Provider (ISP), date and time stamp, referring/exit pages, and possibly the number of clicks. These are not linked to any information that is personally identifiable. The purpose of the information is for analysing trends, administering the site, tracking users' movement on the website, and gathering demographic information.
